- IPV
- MMR
- Varicella
- Hepatitis A
- Hib
- Hepatitis B
- PCV
6th and 7th graders
- MCV4 (meningitis)
- Tdap (tetanus booster)
- HPV (optional)
Insurance
- Please bring a copy of your insurance card and, if available, your vaccination record.
- All insurance coverage, including Medicaid, will be verified.
No Insurance
- Apply "NA" on registration for all insurance questions.
- Patients without insurance coverage will be required to pay out of pocket.
- 18 years and under pay $10 per shot
- 19 years and older pay adult prices, which vary by vaccine
